Abstract
The Leforte I approach provides direct access to the region of the posterior nasal cavity, the sphenoid sinus, pterygo-maxillary fissure, and the clivus. The approach separates the medial and lateral maxillary buttress, allowing inferior displacement of the palate with up to 4 cm of anterior mobilization. At the I'niversity of Iowa during the last four years we utilized this approach in 11 cases. The majority of procedures were treating juvenile nasopharyngeal angiofibroma (eightcases), other tumors included myxoma, clivus chordoma, and recurrent naso-pharyngeal carcinoma. The approach has been combined with anterior craniotomy and lateral rhinotomy when additional access is required. The patient age ranged from 12 to 50 years old. No complications have occurred related to the approach. The approach allows direct access to the mid-portion of the central anterior skull base without cosmetic or functional deficits.
